Through person-centered planning, our options counselors will help map your future. If you choose to remain in your home, we’ll connect you to reliable adult home care, fuel assistance, meal preparation and delivery, and much more. If you’d rather move into a long-term assisted care facility, we’ll help you find a good one. And we work closely with local law firms specializing in estate and financial planning. Learn more here.
Case Management
Helping you at home, we will actively coordinate your care, advocating for you and linking you to all kinds of benefits. Together, we’ll create a sustainable plan tailored to your needs, supporting and respecting your wish to make your own life choices. Learn more here.
Veteran Directed Care
Veteran Directed Care provides veterans of all ages community-based services such as help with bathing, dressing, fixing meals, managing medications, reading, writing letters, and using email. They also provide brief respite for family caregivers. Learn more here.
Volunteer Helpers & Companions
We match volunteers with homebound older adults to provide companionship, or to assist with shopping and other errands. We also have volunteers who will call on the phone regularly to chat and provide a safety check-in. Older veterans may request a veteran volunteer through our Vet-to-Vet program. Occasionally some volunteers may help with minor meal preparation or light housekeeping. They do not provide snow shoveling or personal care, and cannot disburse medication.
