Case Management

Our goal is to enable all our clients to achieve and maintain a meaningful life with dignity. We do this by providing objective, high quality and confidential information, counseling, and assistance to seniors and their families. Case managers actively coordinate in-home care services, help older adults understand and exercise their rights, find information about public benefits, and support informed decisions about quality-of-life issues.  

If you’ve been approved for Medicaid’s Choices for Care or Moderate Needs, you can get your case management from Senior Solutions under those programs. If you need assistance applying for either program, we can help with that too. 

  • Choices for Care provides a package of long-term services and supports to help seniors and adults with physical disabilities become or stay independent while living at home. These services are available to many Medicaid-eligible Vermonters 18 years and over. 
  • The Moderate Needs Group (MNG) Services Program is a component of Choices For Care that provides long-term care services to individuals not meeting the full Choices for Care clinical eligibility. Under the MNG program, participants may receive limited homemaker services from organizations such as the Visiting Nurses Association (VNA) or may hire their own helpers. This program is administered by the Vermont Department of Disabilities, Aging and Independent Living (DAIL).
