Case Management Services
Case management is a collaborative process that helps people achieve their goals by coordinating services and support to meet their needs. Case managers arrange and oversee care plans for clients who qualify for services under the Medicaid Waiver Choices for Care program. This is accomplished by actively coordinating in-home care services, helping older adults to understand and exercise their rights, find information about public benefits, and support informed decisions about quality-of-life issues.
Case managers provide confidential, objective, person-centered assistance to eligible individuals and the families/caregivers to achieve maximum independence.
